After a jolt to your neck, the difficult question is often what to do next. Is it safe to move? Should you rest? And when does pain need medical attention rather than a routine physiotherapy booking?
Start with safety. After a serious accident, call 000. Neck pain following a traumatic accident needs prompt medical assessment, even if symptoms were not obvious at first. New weakness, numbness, difficulty swallowing, changes in vision or loss of bladder or bowel control need urgent medical care. Do not wait for a physiotherapy appointment. Healthdirect explains when to seek help.
What does whiplash mean?
Whiplash describes a neck injury after a sudden movement of the head. Pain, stiffness and headaches can follow. The name alone does not tell you how serious the injury is or which treatment you need. A medical assessment helps rule out problems that need a different kind of care.
Rest or movement: the decision is more specific than that
Once serious injury has been excluded, avoiding all movement is not usually the aim. Australian whiplash guidance supports staying involved in usual activities, making suitable adjustments and using appropriate neck exercises. That does not mean forcing painful movements or copying someone else’s programme. Your starting point should reflect your symptoms and the advice you have received. See SIRA’s treatment guidance.
Tell your clinician which ordinary tasks have changed: checking traffic over your shoulder, sitting at work, sleeping or lifting. Those details make the conversation more useful than simply saying your neck hurts.
What can physiotherapy contribute?
Exercise and advice can form part of whiplash care. Evidence differs between treatments, so a long menu of therapies is not a reason to use them all. The University of Queensland’s recovery resource explains those differences. Ask why a treatment is being suggested, what improvement it is intended to produce and when progress will be reviewed. Read the UQ treatment overview.
You do not need to arrive with a perfect explanation. Bring any medical advice or reports you already have, and a short list of the activities you want to return to. Mention new or changing symptoms rather than assuming they are part of the same problem.
Make the next conversation useful
- What has been ruled out, and is another medical review needed?
- Which activities can I continue, and which need adjusting?
- What should I do if an exercise makes things worse?
- How will we decide whether the plan is helping?
- What would later appointments or treatment cost?
