Turning to check traffic, looking down at your desk, finding a comfortable position in bed: neck pain can make ordinary movements feel like decisions. You may be wondering whether you need a scan, a different pillow or a set of stretches.
The first useful distinction is between a problem suitable for a routine assessment and symptoms that need medical care promptly.
When not to wait for a physio appointment
Call 000 for sudden severe neck pain, neck pain after a traumatic accident, or neck pain with chest pain, breathlessness or sweating. Seek urgent medical care for associated fever, new numbness or weakness, vision changes, confusion, balance problems or bladder or bowel changes. Healthdirect explains these warning signs.
A sore neck does not identify its cause
Neck symptoms can involve muscles, joints, nerves or other conditions. Pain can spread towards the shoulder or accompany a headache. Most people do not need imaging, but your history and examination determine whether further investigation is appropriate.
If there has been no injury or accident, gentle movement within comfort may be appropriate; avoid forcing painful stretches. A clinician can help you decide what to change and whether a medical review is needed.
Bring the everyday problem to the appointment
“I cannot turn comfortably when driving” is a more useful starting point than “my neck is bad”. Tell the clinician when symptoms began, whether they travel into an arm, and what has changed at work, in sleep or during activity. Bring existing reports rather than assuming you must arrange a new scan first.
Ask what the assessment suggests, which findings remain uncertain and what would change the plan. If exercises are proposed, ask to practise them, check the amount and agree what response should prompt a review.
Choose a plan you can use
A desk adjustment is of little practical use if it cannot fit your workplace. Explain your constraints, including driving, shift work, caring responsibilities or equipment you cannot change. Agree on one or two workable steps and a way to judge progress, such as turning, concentrating or sleeping more comfortably.
