Ankle Pain: Injury Checks and Return to Activity

An ankle that hurts after a twist raises a different question from one that repeatedly feels unreliable on uneven ground. You may need an injury checked, help with a return to activity, or an explanation for pain that started without an obvious event.

Before choosing stretches, taping or new shoes, work out which next step fits the situation.

When to seek urgent assessment

After an injury, seek urgent medical care if you cannot bear weight, the ankle looks misshapen, or pain and swelling are severe. A hot, red, swollen ankle with fever needs urgent assessment. A cold, discoloured or numb foot after an injury also needs emergency care. NHS Inform explains ankle warning signs; in Australia use local urgent care or an emergency department, and call 000 for an emergency.

Do not assume every ankle injury is a sprain

Ankle symptoms can involve ligaments, tendons, joints or bone. A clinician can assess the history and movement and decide whether further investigation is needed. If you have had surgery or already have a diagnosis, follow your treating team’s instructions.

If it keeps happening, discuss more than the latest flare

Bring details of previous injuries, what rehabilitation you completed and which activities still feel difficult. Walking on a flat surface may not answer your question about returning to a court sport, trails or a job on uneven ground.

Ask which tasks the assessment will consider and how any programme will progress towards your actual demands. A calendar date alone does not tell you whether you are ready for those tasks.

Make the next step specific

Useful questions include:

  • Does this need medical investigation before rehabilitation?
  • What activity is appropriate while symptoms settle?
  • What should I do if swelling, pain or stability changes?
  • How will we decide when to increase walking, running or sport?

For recurrent pain affecting work, sleep, walking or exercise, a physiotherapy assessment may help you discuss a plan. Leicestershire Partnership NHS Trust explains when persistent ankle problems may benefit from physiotherapy.
